This is a preparation guide, not a universal list of required documents. Your claim type, service history, and VA’s instructions determine what applies. Contact VA or an accredited representative about missing records and deadlines rather than waiting for every box to be checked.
For a disability application, ask about protecting your potential effective date through an intent to file or starting the online application. A phone inquiry alone is not a completed claim or appeal. Read VA’s filing and intent-to-file instructions ↗
1. Prepare for the conversation
This website does not ask for your identity details, medical records, or claim documents. Checkmarks stay in this page only; they are not saved or sent.
Use only the forms that fit your situation. Official online applications may collect the information without a separate PDF. Always open the current VA instructions before completing a form.
New condition or increased rating
VA Form 21-526EZ is the disability compensation application. For an increase, organize evidence of worsening. For a secondary condition, identify the existing service-connected condition and evidence of the relationship. For a presumptive claim, check the applicable diagnosis and service requirements.
Have the prior decision and disputed issues ready. Identify new and relevant evidence, or ask about review based on a change in law. The request uses VA Form 20-0995. Check timing and effective-date consequences with VA or an accredited representative.
Use VA Form 20-0996 when this review is available. Identify the decision, issues, and errors you want reviewed. You cannot submit new evidence in this lane. Check eligibility, the decision-letter deadline, and whether to request an informal conference.
Review VA Form 10182 and your decision letter. Choose the appropriate Board review option; Direct Review, Evidence Submission, and Hearing have different evidence rules. Verify your deadline and get accredited help if you are unsure.
Unable to work because of service-connected disabilities: review Individual Unemployability instructions for Forms 21-8940 and 21-4192; organize work history and employer details.
Mental health conditions involving in-service traumatic events: review VA’s instructions for Form 21-0781 and the information that applies to your claim.
Witness or personal statements: VA describes Form 21-10210 and Form 21-4138 as options; use relevant facts and firsthand observations.
Dependents: review the applicable relationship, school, or dependency documentation before requesting a compensation adjustment.
Leaving active duty: check Benefits Delivery at Discharge timing, separation health assessment, and service-record instructions.
Other special compensation requests: automobile/adaptive equipment, clothing allowance, temporary ratings, and other benefits have their own eligibility and forms.
Use Ask VA through the official contact page, or use VA’s locator to find a regional benefits office. Confirm appointment arrangements, accessibility needs, and what to bring before traveling.
A VA-accredited VSO representative, attorney, or claims agent can help with a claim or decision review. Accredited VSO claims assistance is free. Ask about availability and appointment paperwork.
