Before your visit
- ☐ Confirm the medical condition may qualify for FMP service-connected coverage.
- ☐ Download your FMP benefits letter from VA.gov > My VA > Letters.
- ☐ If requested by the clinic, obtain your VA Rating Code Sheet (not universally required).
- ☐ Bring relevant medical history, medications and allergies; protect private information.
- ☐ Ask whether your provider bills FMP directly.
At the clinic

- ☐ Request clinically accurate notes on medical necessity and relationship to a service-connected disability, when supported.
- ☐ Keep proof of payment if you paid.
- ☐ For hospitalization: discharge summary, and operative report if applicable.
- ☐ For equipment: prescription, description, condition and duration of use.
- ☐ For medications: prescription and detailed pharmacy receipt matching VA requirements.
After treatment
- ☐ File through the VA FMP online claim process, or by approved method using VA Form 10-7959f-2.
- ☐ Attach itemized provider statements, payment proof if paid, and other documentation for applicable care types.
- ☐ VA translates non-English claims; an English copy may speed processing.
- ☐ File within two years of the service date or hospital discharge.
- ☐ Keep your submission confirmation and copies of everything.
